If you are mailing in your registration, please fill out the 3 forms in the Print Registration Forms link on the conference page. Send them in early with your payment.
However, if you are registering online, please simply select 'yes' in the concert drop down list during online checkout. Then put the information about your song selection, etc. in the comments box upon check out (after the credit card info).
